Setting up a Scored Checklist

Scored Checklists allow you to assign points to responses, enabling automatic score calculation as users complete the checklist. Follow these steps to create a scored Checklist Template.

Note: Only Checklist Admins have the permissions to create Checklist Templates.

  1. Create a Checklist Template

    Begin by creating a new Checklist Template and choose Multiple Choice or Multi-Select question types to enable scoring.

  2. Add Scored Questions
    1. Insert a Multiple Choice or Multi-Select question, then check the Scored box.
    2. Assign a numeric score to each answer choice. Scored Checklists allow for full points, half points, or any number up to one decimal place (e.g. 4, 0.5, 0.1).

3. Continue Adding Questions

You can continue adding more scored Multiple Choice questions, along with any additional question types needed for the checklist.

4. Publish the Template

When your template is ready, publish it to make the scored Checklist available for use.

5. Automatic Scoring

As the checklist is completed, the scores for each response will display automatically, and the total score will be calculated in real-time.

Note: If you leave a score blank for any answer option in the template, that response will not be included in the scoring if selected. This can be useful for options like "N/A."
